老年患者腹腔镜胆囊切除术的临床分析

目的探讨老年患者腹腔镜胆囊切除术的临床效果。方法选取2003年11月～2011年11月在本院进行胆囊切除术的老年患者158例，将患者分为对照组和观察组，对照组患者采取开腹手术进行胆囊切除，观察组患者采取腹腔镜进行胆囊切除，分析两组患者手术时间、术中出血量、胃肠恢复时间、住院时间、术后并发症情况。结果和对照组比较，观察组患者手术时间和术中出血量明显减少，胃肠恢复时间和住院时间明显缩短．且胆管损伤、胆漏、胆管狭窄和心力衰竭发生率明显减低，差异有统计学意义（P〈0．05）。结论胆囊镜胆囊切除术是一种安全有效的治疗方法，可以明显改善患者症状，提高患者生活质量，具有显著的临床意义，值得推广。

Clinical analysis of laparoscopic cholecystectomy in elderly patients

Objective To observe the clinical effects of laparoscopic cholecystectomy in elderly patients. Methods A total of 158 elderly patients receiving cholecystectomy in our hospital from November 2003 to November 2011 were randomly divided into control group and observation group. The control group was given laparotomy cholecystectomy and observation group was given laparoseopic cholecystectomy. The operative time, intraoperative blood loss, gastroin- testinal recovery time, hospital stay and occurrence of postoperative complications were analyzed. Results Compared with control group, operative time and intraoperative blood loss of observation group were significantly reduced, gas- trointestinal recovery time and hospital stay significantly shortened, and the rate of occurrence of bile duct injury, bile leakage, bile duct stenosis and heart failure significantly decreased, the differences were statistically significant （P 〈 0.05）. Conclusion Laparoscopic cholecystectomy is a safe and effective treatment method that can significantly improve the patient＇s symptoms and improve their quality of life. It has significant clinical importance and is worthy of promotion.
