人工髋关节置换术后股骨假体周围骨折的治疗

目的总结人工髋关节置换术后股骨假体周围骨折治疗的经验和体会。方法自1996年2月至2004年12月收治人工髋关节置换术股骨假体周围骨折的病人11例，均行手术治疗，包括：单纯行钢丝捆扎的病人2例，普通接骨板固定3例．锁定加压接骨板固定4例．钛缆和接骨板组合应用2例．同时结合植骨3例．石膏外固定1例。结果术后随访6个月-3年．平均10．2个月．10例病人骨折获得骨性愈合．1例出现骨不连．单纯行骨折固定未行股骨柄翻修的病人Harris评分无明显影响，同时行翻修的病人Harris评分明显升高。结论全面术前计划，充分考虑病人骨量丢失，选择合适坚强的内固定，术中仔细操作。

Treatment of femur periprothetic fracture after hip arthroplasty

Purpose To summarize the experience of treatment of femur periprothetic fracture after hip arthroplasty. Methods From February 1996 to October 2000,eleven patients with femur periprothetic fracture after hip arthroplasty were studied.All patients were treated with internal fixation.Two fractures were bundled up merely with steel-wire,three cases were fixed with common plate,four cases with lockig compress plate(LCP),two cases with tiantium cable combined with plate.Three cases were grafted,and one case combined with external plaster fixation at the same time. Results All patients were followed up for six months to three years with average of 10.2 months.Bony heal was obtained in 10 patients,bone ununion occurred in 1 patient.The Harris Score did not change significantly in those who received fracture internal fixation merely without femur stem reversion,and increased significantly in those who received fixation and femur stem reversion at the same time. Conclusions Plan carefully before operation,recognize the lose of bone mass sufficiently and select firm internal fixation as well as operate carefully.
