不同年龄阶段的高龄女性体外受精-胚胎移植的妊娠结局分析

[目的]探讨年龄对高龄女性体外受精-胚胎移植结局的影响.[方法]收集2013年1月至2015年12月在本院生殖中心行新鲜胚胎移植的高龄不孕症患者1559例共1559个周期,按患者年龄分为35~39岁组(914个周期,A组)、40~44岁组(542个周期,B组)、45岁及以上组(103个周期,C组).比较各组的受精率、卵裂率、优胚率、取消率、种植率、妊娠率、活产率和流产率.[结果]三组的受精率、卵裂率、优质胚胎率比较差异均无统计学意义(P>0.05);A组、B组、C组取消率、流产率依次升高,组间比较差异有统计学意义(P<0.05);种植率、妊娠率、活产率分别依次降低,组间比较差异有统计学意义(P<0.05).[结论]在高龄女性中,年龄是影响IVF-ET成功的重要因素,建议45岁以上寻求赠卵治疗.

Pregnancy Outcome Analysis of in Vitro Fertilization-Embryo Transfer in Elderly Women of Different Ages

[Objective] To investigate the influence of age of elderly female patients on the outcome of in vitro fertilization-embryo transfer(IVF-ET).[Methods]In this retrospective analysis,a total of 1559 elderly infertility patients who received fresh embryo transfer between Jan 2013 and Dec 2015 were analyzed.The eld-erly patients,with a total of 1559 cycles,were divided according to the ageinto 3 groups:ages 35~39(group A,914 cycles),ages 40~44(group B,542 cycles),and ages ≥45(group C,103 cycles).Results of insemi-nation rate,clearage rate,rate of high quality embryo,cycle cancelation rate,implantation rate,pregnancy rate,live birth rate and abortion rate were compared among the three groups.[Results]There was no signifi-cant difference in the insemination rate,clearage rate and the rate of high quality embryo among three groups (P>0.05).In the order of group A,B to C the cycle cancelation rate(27.13%、43.73%、63.11%,respective-ly),implantation rate(27.63%、13.29%、1.39%,respectively),pregnancy rate(46.83%、29.51%、2.63%,re-spectively),live birth rate(33.78%、15.41%、0%,respectively)and abortion rate(22.44%、38.67%、100%, respectively)increased or decreased;there were significant differences(P<0.05)between each group.[Con-clusion]In elderly infertility women,the age of the patients has an important effect on the success of IVF-ET.
