序贯放化疗与同期放化疗治疗进展期胃癌的临床疗效比较

目的]探讨进展期胃癌根治术后序贯放化疗与同期放化疗的临床疗效.方法]胃癌根治术术后患者160例,随机分为两组,观察组采用序贯放疗,即辅助化疗4周期后联合调强放疗,对照组采用同期放化疗,比较两组疗效.结果]观察组1年、2年、3年生存率分别为87.50％ (70/80)、76.25％(61/80)、53.75％(43/80),明显高于对照组73.75％、60.oo％、37.50％(P＜0.05).观察组发热,胃肠道反应,肝肾功能异常等并发症发生率与对照组比较差异无显著性(P＞0.05).观察组3年复发21例,复发率为26.25％(21/80),明显低于对照组50.o％(40/80),且差异有显著性(P＜0.05).结论]对于进展期胃癌的术后治疗,序贯化放疗比同期放化疗临床效果更为显著,且复发率低,值得推广应用.

Clinical Efficacy of Sequential Chemoradiotherapy and Concurrent Chemoradiotherapy in the Treatment of Advanced Gastric Cancer

Objective]To investigate the clinical efficacy of sequential chemoradiotherapy and concurrent chemoradiotherapy in the treatment of advanced gastric cancer.Methods] One hundred and sixty patients with gastric cancer after radical operation were randomly divided into two groups,the observation group was treated with adjuvant chemotherapy after the 4 cycles of intensity modulated radiation therapy,the control group was treated with radiotherapy and chemotherapy at the same time,the efficacy of the two groups were compared between the two groups.Results]The survival rate of the observation group was 87.50％,76.25％,53.75％ (43/80) respectively,which were significantly higher than those in the control group (73.75％,60％,37.50％) (P ＜0.05);There was no significant difference in the incidence of complications such as fever,gastrointestinal reaction,liver and kidney dysfunction in the observation group compared with the control group (P ＞0.05).In the observation group,the recurrence rate was 26.25％ (21/80),which was significantly lower than that in the control group (40/80),and the difference was significant (P ＜0.05).Conclusion]In the treatment of advanced gastric cancer,sequential radiotherapy is more effective than the concurrent radiotherapy and chemotherapy,and the recurrence rate is low,so it is worthy of popularization and application.
