17β-雌二醇对大鼠蛛网膜下腔出血后迟发型脑血管痉挛的作用

摘    要:目的研究17β-雌二醇(E2)对蛛网膜下腔出血(SAH)后迟发型脑血管痉挛(DCV)的抑制作用。方法雄性Wistar大鼠50只随机分为5组:①空白组,②假穿刺组,③SAH组,④SAH+E2组,⑤SAH+安慰剂组。采用酶联免疫吸附法(ELISA)检测血浆中内皮素-1(ET-1)含量,末端脱氧核苷酸转移酶介导的生物素脱氧尿嘧啶核苷酸缺口末端标记法(TUNEL)检测颞叶神经元凋亡情况,通过测定基底动脉血管横截面积判断脑血管痉挛情况。结果实验结果显示SAH后7 d SAH+E2组基底动脉横截面积与SAH组和SAH+安慰剂组相比明显变大(P<0.01);与SAH组和SAH+安慰剂组相比,SAH+E2组血浆ET-1浓度明显减少(P<0.01);TUNEL染色显示SAH+E2组颞叶皮质神经元凋亡程度较SAH组和SAH+安慰剂组显著性减轻。结论持续给予E2维持其生理浓度可以有效预防SAH后迟发型脑血管痉挛,部分可能与E2可以抑制ET-1的产生有关。

Abstract:Objective To study the effect of 17beta-estradiol(E2) on delayed cerebral vasospasm(DVC) after subarachnoid hemorrhage(SAH) in rats.Methods Fifty Wistar male rats were randomly divided into five groups: non-treatment group,control group,SAH group,SAH plus E2 group,and SAH plus placebo group.The content of endothel-1(ET-1) in blood was determined by enzyme-linked immunosorbent assays(ELISA).Terminal deoxynucleotidyl transferase-mediated biotinylated deoxyuridine triphoephate nick end labeling assay(TUNEL) was used to observe the apoptosis of temporal cortex.The cross sectional area of each basilar lumen was measured to evaluate the degree of cerebral vasospasm.Results The cross sectional area of the basilar artery in SAH+E2 group was larger than that in SAH or SAH + placebo group(P0.01);the concentration of ET-1 in SAH+E2 group was lower than that in SAH or SAH + placebo group(P0.01);TUNEL revealed that the apoptosis rate of the temporal cortex in SAH + E2 group was lower than that in SAH or SAH + placebo group.Conclusion These results suggest that continuous administration of E2 to maintain the physiological level prevents the cerebral vasospasm following SAH,which may be partially related with its inhibitory effect on the production of ET-1.
