腋下小切口在肺部手术中的应用

目的为了改进传统开胸手术切口，探索腋下小切口在肺部手术中的应用。方法采用腋下小切口（切口平均长度１１．７ｃｍ）对１０例肺部肿瘤患者施行了肺叶楔型切除术、肺叶切除术和肺癌根治性手术。结果１０例患者术中及术后均未发生合并症，术后恢复明显加快。结论腋下小切口伤口和肌肉损伤小，出血少，手术时间短；切口不横断胸壁的肌肉、不切除肋骨和不缝扎肋间神经，故术后伤口疼痛持续时间短而轻；切口不牵拉肩胛骨，术后肩关节和上肢活动受影响小、恢复快。

The Application of Subaxillary Small Incision Thoracotomy in the Pulmonary Surgery

Objective To search for the clinical feasibility and advantages of subaxillary small incision thoracotomy in the pulmonary surgery. Methods Pulmonary wedge resection and lobectomy by using subaxillary small incision thoracotomy (the average length of incision was 11.7cm) were performed on 10 patients with lung cancer. Results There was no intraoperative and post operative complications, and the post operative convalescence was much shortened. Conclusion Subaxillary small incision thoracotomy through the intercostal space reduces bleeding and trauma to muscles, intercostal nerves and rib so that the post operative pain is much reduced, hence the post operative complication reduced and the convalescence is smooth. This approach is applicable to some selected patients for lung surgery.
