全肺静脉畸形引流的彩色多普勒诊断

本文对照分析14例全肺静脉畸形引流（TAPVC）和13例房缺的超声结果，认为右心的改变对诊断TAPVC无特异性，左房形态及大小变化，肺静脉回流左房图象消失和房水平明显右向左分流对诊断有较大的价值。肺静脉引流部位的判断，采色多普勒和X线右心造影符合率（100％）高于二维超声。对心内并发畸形的诊断右心造影（90％）低于超声（100％）。

Color Doppler Image for Diagnosis of Total Abnormal Pulmonary Venous Connection

Echocardiographic results of 14 patients with total abnormal pulmonary vein connection (TAPVC) were analyzed. 13 simple ASD surgically proved cases used as control. It was considered the changes of right heart are not specific for diagnosis of TAPVC; but the changes of left atrial shape and size, absence imaging of pulmonary vein returning to the left atrium and presence of right atrium to left atrium shunt are valuable for diagnosis. For determination of the site of returning pulmonary veins, the accuracy of angiography (100%) is equal to that of color Doppler images and higher than that of two-dimensional echocardiography (78.6%). On the other hand, the accuracy of angiography (90%) is lower than those of two-dimensional echocardiography and color Doppler images (100%) for diagnosis of intracardiac complicated abnormalities.
