循证护理对留置尿管患者拔管后自行排尿的影响

目的探讨循证护理对提高留置尿管患者拔管后自行排尿成功率的应用效果。方法选取2010年1月-2012年12月留置尿管的住院患者375例，按入院先后顺序随机编号，分为A组(常规组)、B组(注药组)、C组(循证组)各125例。A组采用常规拔尿管的方法，B组采用注药拔尿管的方法，C组采用循证护理的方法拔出尿管，比较3组患者拔管时的疼痛程度及拔管后的首次自行排尿成功率。结果A组无痛23例，B组65例，C组103例，C组拔管时疼痛程度低于A组和B组，C组拔管后首次自行排尿成功率(88.8%)高于A组(32.8%)和B组(66.4%)。结论循证护理结合临床实践能提供最佳的护理措施，减轻留置尿管患者拔管时的疼痛，提高首次自行排尿成功率。

Effects of evidence based nursing on urination of patients with indwelling catheter after extubation

Objective To study the effects of evidence based nursing on urination of patients with indwelling catheter after extubation .Methods Totally 375 hospitalized patients with indwelling catheter in our hospital from January 2010 to December 2012 ,were divided into group A （n=125） , group B （n=125） ,and group C （n=125） ,according to the sequence of admission .The group A was given conventional method to pull out the catheter ,the group B was given drug injection before extubation ,the group C was given evidence-based nursing at the base of group B .The pain degree and the success rate of urination by themselves for the first time after extubation of 3 groups were compared .Results The painless patients in group A was 23 ,group B was 65 ,group C was 103. The pain degree of group C were lower than that of group A and group B .The success rate to urinate by themselves for the first time in group C （66 .4% ） were higher than that in group A （32 .8% ） and group B （88 .8% ） .Conclusion Evidence-based nursing combined with clinical practice ,can provide effective nursing measures ,reduce the pain during extubation ,improve the success rate to urinate by themselves for the first time .
