慢性乙型肝炎患者肝组织炎症与肝功能和乙型肝炎病毒标志物的关系

目的：探讨慢性乙型肝炎（乙肝）和活动性肝硬变肝组织病变与乙肝病毒标志物（HBVM）和肝功能的关系。方法：在Knodell的乙肝肝组织炎性活性指标（HAI）分类上采用3次密码读片。用常规的方法检测血清胆红素（SB）、丙氨酸转氨酶（ALT）、天冬氨酸转氨酶（AST）、白蛋白／球蛋白（A／G）比率，肝内HBsAg,HBcAg检测用PAP法。结果：SB、ALT、AST值随HAI记分值的增高而增高，慢性肝炎，活动肝硬变患者血清A、G的含量及A／G随HAI记分值的增高而变化，结论：HAI能客观定量的反映肝组织病变的程度，为疾病的诊断，治疗和预后判断提供一种实用的病理指标，并有利于微机图像阅片和统计学分析。

Study on Relationship between Liver Histological Activity Index and Liver Function or HBVM in the Patients with Chronic Hepatitis B

Objective: To investigate the relationship between liver histopathologic activity and HBV markers or liver function in patients with chronic hepatitis B and active liver cirrhosis. Methods: Histological activity index (HAI) classification of Knodell was used to analyze liver tissues by code reading thrice. SB, ALT, AST and A/G were detected by routine tests. HBsAg and HBcAg were tested by PAP. Results: The levels of serum SB, ALT, AST were increased as HAI scores elevated. Serum albumin, globulin levels and A/G ratio changed with HAI scores increased in patients with chronic hepatitis and active liver cirrhosis. Conclusion: It suggested that HAI could reflect liver pathohistologic degree objectively and quantitatively, acting as a practical pathological indicator for diagnosis, treatment and prognosis of chronic hepatitis and active liver cirrhosis, helpful for computer reading section and statistical analysis.
