PCNA对早期乳腺癌预后意义的评估

目的;探讨增殖细胞核抗原在早期乳腺癌的表达情况及其与预后的关系。方法：对1974年至1985年98例手术治疗的Ⅰ期乳腺癌的原发灶进行增殖细胞核抗原（PCNA）免疫组织化学法检测,并与临床病理资料进行比较分析。结果：PCNA在Ⅰ期乳腺癌表达率为75．5％。随PCNA表达强度的增加无病生存率（5a,10a,15a)呈下降的趋势,但无显著差异;PCNA高表达组淋巴结转移率（40．7％）明显高于低表达组（18．4％,P＜0．05）;PCNA与病理发级,肿瘤大小及组织类型之间未见有相关性。结论：在1期乳腺癌PCNA表达与淋巴结转移呈正相关,与预后呈负相关趋势,是估计预后的有用指标。

Evaluation of prognosis and expression of PCNA in the early breast cancer

Objective: To investigate the expression of proliferating cell nuclear antigen(PCNA)in early stage breast cancer and to evaluate the prognostic significance of PCNA. Methods: A total of 98 patients with stage I breast cancer were selected to this study from the years 1974 to 1985.PCNA was analyzed by immunohistochemistry in primary lesions in these patients,The results were analyzed to compare with clinicopathological factors in this group of patients.Results:The expression rate of PCNA was 75.5% in early breast cancer.There was a trend of increase of expression level of PCNA compared with decrease of 5-year,10-year,15-year disease-free survival,but which hadn't significant difference.High expression group of PCNA and low expression group of PCNA had significant difference in lymph node invol ement(40.7% and 18.4%)(P<0.05). Disease-free survival is not connected with PCNA expression in tumor grade,tumor size and histopathological type.Conclusion:PCNA expression was significantly related to the lymph node invol ement and prognosis and may be used as one of the prognostic indicators in early breast cancer.
