经中鼻道上颌窦开窗术

研究目的为了提高慢性上颌窦炎的治疗效果,探讨简便有效的手术方式。研究背景经典的Caldwell-Luc术式疗效不理想,常伴有并发症,需寻求新的治疗手段。研究方法根据鼻解剖及窦粘膜生理功能,进行以上颌窦自然孔为中心的中鼻道“开窗”,并在直视下清除窦内病变、观察疗效。结果通过14例19侧手术体会,良好率90％,好转率10％。认为此术式损伤小,无并发症,符合窦粘膜生理功能,疗效较高。结论上颌窦开窗术可取代上颌窦根治术及下鼻道开窗术,优于单纯自然孔开大术。

Fenestration of middle nasal mcatus in treat-ing maxillary sinusitis

Objective In order to achieve good results of treating chronic maxillary sinusitis we have searched a simple, more effective method of operation.Background There is more complications in traditional Caldwell-Luc operation. It is necessary to seek a new operation.Method This method of fenestration of middle nasal meatus gets hold of drainage easily and the area of pathological changs is excised in sight directly.Results Through 19 lateral operation of 14 cases, 90% effective, 10% turned for the better. This operation is less injurious and accords with physiological function of mucosa.Concclusion This operation may replace radical maxillary sinusotomy and intranasal antrostomy. It is more advanced than the enlarged operation of sinus maxillary aperture.
