体位复位联合经皮置钉技术治疗胸腰椎重度压缩性骨折

摘    要:目的 分析胸腰椎重度压缩性骨折采取体位复位联合经皮置钉技术治疗的临床效果.方法 本次试验将医院2015年1月—2018年12月期间收治的60例胸腰椎重度压缩性骨折患者作为研究对象,术前在给予全身麻醉后行体位复位,获取满意的复位后,采取经皮置钉手术.术后进行1年随访,分析患者手术前后ODI指数、VAS评分及各项影像学指标变化情况.结果 与术前比较,患者术后1个月、12个月的ODI指数和VAS评分均显著降低,(P<0.05);与术前比较,患者经体位复位后、撑开钉棒后,后凸Cobb角、前缘、中部椎体高度丢失率均取得显著改善,(P<0.05).结论 采用体位复位联合经皮置钉技术治疗胸腰椎重度压缩性骨折患者,具有较小的创伤性,能够良好矫正伤椎,确保治疗的安全性.

Abstract:Objective To analyze the clinical effect of thoracolumbar vertebrae severe compression fractures treated with postural reduction combined with percutaneous nail placement.Methods In this trial,60 patients with severe thoracolumbar vertebral fractures treated in our hospital from January 2015 to December 2018 were taken as the research subjects.Postoperative general anesthesia was performed to reduce the body position before surgery.Percutaneous nailing.A one-year followup was performed to analyze the changes of ODI index,VAS score and various imaging indicators before and after surgery.Results Before surgery,the ODI index and VAS score of the patients were significantly reduced at 1 and 12 months after operation(P<0.05).Before surgery,the patients were compared with Cobb after the position reduction,after the nail rod was opened,and the Cobb was convex The angle,anterior margin,and central vertebral height loss rates were significantly improved(P<0.05).Conclusion The use of postural reduction combined with percutaneous nail placement in the treatment of patients with severe compression fractures of the thoracolumbar vertebrae is less traumatic and can be used to correct the injured vertebra and ensure the safety of treatment.
