应用残疾调整生命年测量恶性肿瘤疾病负担

[目的]评价杭州市区恶性肿瘤的疾病负担及其变化情况.[方法]应用2000年～2002年杭州市肿瘤报病资料及死因监测报告资料,采用残疾调整生命年(DALY)测量恶性肿瘤的疾病负担.[结果]杭州市区恶性肿瘤残疾调整生命损失年数(DALY)2000年为65 847.81人年,2001年为54 339.94人年,2002年为55 156.07人年.近3年,支气管肺癌、肝癌、结直肠癌和胃癌DALY虽在下降,但绝对数仍是很大;乳腺癌和宫颈癌的DALY在上升.因恶性肿瘤造成的早死而引起的生命损失年数(YLL)逐年下降,而伤残损失健康生命年(YLD)逐年增加.[结论]支气管肺癌、肝癌、肠癌和胃癌仍是影响杭州市区居民健康的主要危险肿瘤类型.乳腺癌和宫颈癌的变化趋势应引起相关人员的重视.

Measurement of the Burden of Malignant Tumor with Disability-adjusted Life Years

To estimate the burden and changing tendency of malignant tumor.Based on the data of cancer registry and death cause surveillance from 2000 to 2002 in Hangzhou,the burden of malignant tumor was measured with disability-adjusted life years (DALY).Disability-adjusted life years(DALY)of malignant tumor was 65 847.81 person year in 2000,54 339.94 person year in 2001,55 156.07 person year in 2002 in Hangzhou. In recent 3 years,the DALY of lung cancer,liver cancer,intestine cancer and gastric cancer were decline, but the DALY of these cancer were larger.And the DALY of breast cancer and cervical cancer were rising.The years of life lost (YLL) caused by malignant tumor was decline, but the years lived with disability (YLD) was rising.[Conclusion]Lung cancer,liver cancer,intestine cancer and gastric cancer are also main cancers effecting citizen health.And the control and prevention of breast cancer and cervical cancer should be emphasized.
