全胸腔镜与小切口开胸肺叶切除术治疗早期非小细胞肺癌的疗效比较

目的：比较全胸腔镜与小切口开胸肺叶切除术治疗早期非小细胞癌的临床疗效。方法收集100例早期非小细胞肺癌患者，按照患者意愿选择分为观察组（n＝53）和对照组（n＝47）。观察组于全胸腔镜下行肺叶切除术，对照组行小切口开胸肺叶切除术，比较两组手术情况及1年和3年生存率和复发率。结果100例患者均顺利完成手术。治疗后观察组与对照组比较，患者的手术时间和术中淋巴结清扫数目比较差异无统计学意义（P＞0．05），而术中出血量、术后胸腔引流量、术后镇痛时间及术后住院时间比较差异有统计学意义（ P＜0．05）。观察组术后疼痛VAS评分明显低于对照组。两组患者1年、3年肿瘤复发率和生存率比较无显著差异（P＞0．05）。结论全胸腔镜下行肺叶切除术可缩短住院时间、减少术中出血量、减轻术后疼痛、患者恢复快，是临床治疗早期肺癌的有效手段。

Comparison of Clinical Efficacy of Complete Video-assisted Thoracoscopic and Mini-thoractomy for Early Non-small Cell Lung Cancer

Objective To compare the clinical efficacy of complete video-assisted thoracoscopic and mini-thoractomy for early non-small cell lung cancer.Methods 100 cases of early non-small cell lung cancer were selected and grouped into the ob-servation group(n=53)with complete video-assisted thoracoscopic and the control group(n=53) with mini-thoractomy.The clin-ical efficacy was compared between the 2 groups.1-and 3-year recurrence rates and survival rates after surgery were compared. Results 100 patients were completed successfully by surgery.Compared with the observation group and the control group,opera-tion duration and number of lymph node dissection in lung cancer showed no statistically difference(P>0.05).The intraoperative bleeding,days of postoperative analgesia,volume of chest tube drainage and the length of postoperative hospital stay of patients showed statistically difference(P<0.05).The postoperative pain VAS scores of the observation group was significantly lower than the control group.1-,3-year recurrence and survival rates between the 2 groups had no significant difference ( P>0.05) .Conclu-sion Thoracoscopic lobectomy can shorten the length of postoperative hospital stay,reduce intraoperative bleeding volume and postoperative pain with fast recovery,it is an effective treatment for early lung cancer.
