儿童急性白血病医院感染因素的Meta分析

目的 通过对国内有关儿童急性白血病(acute leukemia,AL)医院感染因素的病例对照研究结果进行Meta分析,以期找出AL医院感染的危险因素及防治对策.方法 从中国生物医学文献数据库、中国学术期刊全文数据库和万方数据库中检索1990-2008年国内已发表的相关文献,选择符合标准的文献进行Meta分析.结果 经过分析AL类型、中性粒细胞数量、骨髓增生程度、化疗阶段因素差异有统计学意义(P＜0.05),性别、预防使用抗生素因素差异无统计学意义(P＞0.05).结论 急性非淋巴细胞白血病、中性粒细胞数量小于1.0x109/L、骨髓增生抑制、缓解化疗阶段的AL患儿发生医院感染的危险性高,而性别、预防使用抗生素不是AL患儿发生医院感染的危险因素.

Meta analysis of nosocomial infection factors in childhood acute leukemia

Objective To perform the Meta analysis on the results of domestic comparative studies on related nosocomial infection factors in childhood acute leukemia(AL) in order to find out the risk factors of AL and the countermeasures of prevention and treatment.Methods To retrieve the published related literature at home during 1990-2008 from CMBdisc,CHKD and Wanfang Databases.The literatures meeting the criterion were performed the Meta analysis.Results The analysis showed that AL types,absolute neutrophil count,bone marrow hyperplasia degree,chemotherapy had statistical difference.But the difference in sex and preventive antibiotics utilization had no statistical significance.Conclusion The children patients with acute nonlymphocytic leukemia(ANLL),number of neutrophils<1.0×109/L,myeloproliferative suppression and remission stage of chemotherapy could have higher risk of nosocomial infection.Sex and preventive antibiotics use are not the risk factors causing the occurrence of nosocomial infection.
