ADA、CEA及PFLMNF在胸腔积液鉴别中的临床意义

摘    要:目的:分析腺苷脱氨酶(ADA)、癌胚抗原(CEA)及淋巴细胞微核细胞率(PFLMNF)在胸腔积液鉴别中的意义。方法根据病理类型将本院收治的68例胸腔积液患者分为恶性肿瘤性胸腔积液组(33例)和结核性胸腔积液组(35例),分别对两组患者的胸腔积液ADA、CEA的表达水平及PFLMNF进行检测。结果恶性肿瘤性胸腔积液组患者的ADA值低于结核性胸腔积液组(P〈0.05),CEA及PFLMNF值均高于结核性胸腔积液组(P〈0.05)。恶性肿瘤性胸腔积液组患者的ADA、CEA及PFLMNF阳性率均高于结核性胸腔积液组(P〈0.05),ADA、CEA及PFLMNF的联合阳性率高于结核性胸腔积液组(P〈0.05);恶性肿瘤性胸腔积液组患者ADA、CEA及PFLMNF的联合阳性率均高于三项指标单独阳性率(P〈0.05)。结论ADA、CEA及PFLMNF的联合检测对良恶性胸腔积液鉴别诊断具有重要意义。

The clinical significance of ADA,CEA and PFLMNF in identification of pleural effusion

Abstract:Objective To analyze the clinical significance of adenosine deaminase (ADA),carcino-embryonic antigen (CEA) and PFLMNF in identification of pleural effusion. Methods 68 cases of patients with pleural effusion were divid-ed into malignant tumor pleural effusion group (33 cases) and tuberculous pleural effusion group (35 cases) according to the pathological type.The expression of ADA,CEA and PFLMNF were detected in two groups. Results The ADA of ma-lignant tumor pleural effusion group was lower than that of tuberculous pleural effusion group (P〈0.05),CEA and PFLMNF were higher than those of tuberculous pleural effusion group (P〈0.05).The positive rate of ADA,CEA and PFLMNF in malignant tumor pleural effusion group were higher than those in tuberculous pleural effusion group (P〈0.05);the joint positive rate of ADA,CEA and PFLMNF in malignant tumor pleural effusion group was higher than that in tu-berculous pleural effusion group (P〈0.05);the joint positive rate of ADA,CEA and PFLMNF in malignant tumor pleural effusion group was higher than that in three indicators of separate positive rate (P〈0.05). Conclusion ADA,CEA and PFLMNF joint detection has significance in differential diagnosis of benign and malignant pleural effusion.
