一期多通道经皮肾镜取石术对患者肾功能的影响

目的]探讨一期多通道微创经皮肾镜取石术（MPCNL）对患者肾功能的影响.方法]对57例鹿角形结石患者行一期多通道MPCNL治疗.采用Cockroft-Gault方程计算肾脏的肾小球滤过率（GFR）,比较术前、术后24 h及术后2周GFR值,观察患者手术前后肾功能的变化情况.结果]57例患者术后24 h肾脏GFR由术前的（113.86±36.14）mL/min下降至（84.27±29.15）mL/min,下降约26%,两者相比较差异有统计学意义（P＜0.01）.术后2周GFR值为（112.38±35.79）mL/min,与术前相比,差异无统计学意义（P＞0.05）.结论]一期多通道MPCNL术对鹿角形结石患者的肾功能无明显的影响,是治疗鹿角形结石一种安全的方法

Effect of One-stage Multi-tract MPCNL on Renal Function

Objective] To explore the effect of one-stage multi-tract percutaneous nephrolithotomy（MPC-NL） on renal function. Methods] A total of 57 patients with stag-horn calculi were treated with one-stage multi-tract MPCNL. Glomerular filtration rate（GFR） was calculated by using Cockroff-Gault formula. GFR before, 24 hours and 2 weeks after MPCNL were compared. The change of renal function before and after operation was observed. Results] Renal GFR of 57 patients decreased from （113.86±36. 14）mL/min before operation to （84.27±29.15）mL/min after operation. The decrease rate of GFR was 26%. There was significant difference in GFR between before and after operation（ P〈0. 001）. GFR at 2 weeks after operation was （112.38±35.79）mL/min, but there was no significant difference between before and after operation（ P〉0.05）. Conclusion] One-stage multi-tract MPCNL has no obvious effect on renal function of patients with stag-horn calculi and is a safe treatment option for stag horn calculi.
