舒芬太尼和芬太尼对腹腔镜胆囊切除术患者全麻苏醒质量的比较

[目的]比较舒芬太尼和芬太尼对腹腔镜胆囊切除术全麻苏醒质量的影响.[方法]将110例择期行腹腔镜胆囊切除手术的患者随机分为舒芬太尼（S）组和芬太尼（F）组,每组55例.S组以咪达唑仑0.1 mg/kg、舒芬太尼0.4 μg/kg、丙泊酚2 mg/kg、维库溴胺0.12 mg/kg诱导插管;F组,以咪达唑仑0.1 mg/kg、芬太尼4 μg/kg、丙泊酚2 mg/kg、维库溴胺0.12 mg/kg诱导插管.以丙泊酚0.5 mg/（kg·h）和瑞芬太尼0.25 mg/h维持麻醉.比较两组术后患者的呼吸恢复时间、呼唤睁眼时间、气管导管拔出时间及术后的疼痛语言（VRS）分级评分和镇静（SS）评分及苏醒期内不良反应的发生情况.[结果]F组患者术后呼吸恢复时间、呼唤睁眼时间、气管导管拔出时间均长于S组,且两组相比较差异有显著性（P〈0.05）,F组术后躁动发生率、疼痛发生率显著高于S组（P〈0.01）,S组镇静强度显著强于F组（P〈0.01）.[结论]与芬太尼全身麻醉相比,使用舒芬太尼的患者苏醒期更平稳

Comparison of Effects of Sufentanil vs Fentanyl on the Quality of the Recovery of Patients Undergoing Laparoscopic Cholecystectomy after General Anesthesia

[Objective] To observe the clinical occurrence of restlessness during the recovery of patientsundergoing laparoscopic cholecystectomy after general anesthesia with sufentanil and fentanyl, and to compare the effects of sufentanil vs fentanyl on the recovery of patients undergoing laparoscopic cholecystectomy after general anesthesia. [Methods] Totally 110 patients scheduled for laparoscopic cholecystectomy were randomly divided into sufentanil group（group S） and fentanyl group（group F） with 55 cases in each. Group S was given midazolam 0. lmg/kg, sufentanil 0.4pg/kg, propofol 4mg/kg and vecuronium 0.12mg/kg for the induced in tubation. Group F was given midazolam 0. lmg/kg, fentanyl 4/μg/kg, propofol 2mg/kg and vecuronium 0.12 mg/kg by the induction of intubation. Propofol 0.5mg/（kg · h） and remifentanil 4/μg/（kg· h） were given for anesthesia maintenance. The time of regaining spontaneous breathing, the time of calling eyes open, extubation time, postoperative verbal rating scale（VRS） and sedation score（SS） after operation were analyzed. The side effects during the recovery were recorded. [Results] The time of regaining spontaneous breathing, the time of calling eyes open and extubation time in group F were longer than those in group S, and there were sig nificant differences between two groups（ P 〈0.05）. The incidence of restlessness and pain after surgery in group F were markedly higher than those in group S（ P 〈0.01）. But the sedation effect of group S was signif icantly better than that of group F（ P〈0.01）. [Conclusion] The recovery of patients after general anesthesia with sufentanil is more stable than that after general anesthesia with fentanyl.
