超声引导高强度聚焦超声治疗腹壁子宫内膜异位症（附28例报告）

【目的】探讨超声引导下高强度聚焦超声（HIFU）对腹壁子宫内膜异位症的治疗效果。【方法】超声引导HIFu治疗门诊腹壁子宫内膜异位症患者28例，治疗系统发射频率0．9MHz，焦距120mm，治疗头直径200mm，治疗功率240～400W，扫描时间1～10s／m3，间隔时间0．3～0．4s。在治疗过程中，连续监测患者血压、脉搏、呼吸，并观察患者疼痛情况。【结果】病灶内血流信号消失患者26例，信号减少者1例，无变化者1例；26例患者临床症状消失，1例患者临床症状减轻，1例患者临床症状无缓解，无皮肤灼伤的不良反应及严重并发症发生。【结论】超声引导HIFU治疗腹壁子宫内膜异位症，具有初步的临床应用价值。

Clinical Application of Ultrasound-guided High-intensity Focused Ultrasound in the Treatment of Abdominal Wall Endometriosis

Objective] To explore the efficacy of ultrasound-guided high-intensity focused ultrasound（HIFU） for the treatment of abdominal wall endometriosis （AWE）. Methods] Twenty-eight outpatients with AWE were treated by ultrasound-guided HIFU. The transmitting frequency of therapeutic system was 0.9 MHz and the focal distance was 120mm. The diameter of therapeutic transducer was 200mm and the power was 240-400W. The scanning time was 1; 10s/m3 and the interval time was 0.3;0.4s. Blood pressure, pulse and respiration of patients during the treatment were monitored continuously. The tolerable pain of patients was observed. Results] The disappearance of blood flow signal was detected in lesions of 26 patients and the decreasing of blood flow signal was found in one case. One case had no change of blood flow signal. The disappearance of clinical symptoms was found in 26 patients. The clinical symptoms of one case were alleviated. One case had no relief of clinical symptoms. No adverse reaetion such as skin scorch and other severe complications occurred. Conclusion] US-guided HIFU for the treatment of AWE has preliminary clinical value.
