老年人右半结肠癌并存阑尾炎误诊26例临床分析

目的　分析老年人右半结肠癌并存阑尾炎误诊的原因及预防措施。方法　回顾性总结了 1993年 7月至 2 0 0 1年 12月我科收治 2 6例该类患者的诊治情况。结果　 12例患者单纯行阑尾切除术 ,术后复诊合并有右半结肠癌而再次手术 ;8例患者行阑尾切除或阑尾周围脓肿切开引流时发现为右半结肠肿瘤 ,故术中又行右半结肠切除或回结肠短路手术 ;另 6例患者术前误诊为阑尾炎性包块而采取保守治疗 ,经确诊后方行右半结肠癌根治术。结论　对老年人更应详细询问病史和仔细体检 ,术中应注意探查邻近结肠 ,尤其是回盲部 ,避免右半结肠肿瘤误诊和漏诊的发生

Study on the misdiagnosis of right hemicolon carcinoma combined appendictis in 26 cases of senile patients

Objective To analysis the reasons and prevention measures of right hemicolon carcinoma misdiagnosed as appendicitis in senile patients.Methods A respective analysis was made in 26 cases of patients from July 1993 to December 2001.Results 12 patients were only given appendectomy and given right hermicolectomy after being diagnosed correctly. 8 patients were given right hemicolectomy or ileotransverse colostomy during the operation of appendectomy.6 patients were given conservative treatments before they were diagnosed correctly.Conclusions In order to prevent the misdiagnosis and missed diagnosis of colon carcinoma in old patients,we should get history of illness in detail and make physical examination carefully to them. On the other hand, they should be given careful exploration of near-by colon in the operation.
