持续腰椎悬吊牵引治疗胸腰椎压缩性骨折

摘    要:
目的:探讨持续腰椎悬吊牵引治疗胸腰椎骨折的临床疗效。方法:2001-2010年使用持续腰椎悬吊牵引治疗胸腰椎骨折140例,资料完整的有120例,其中男47例,女73例;年龄21~80岁,平均年龄55岁。伤后就诊时间30min~6d。骨折部位:T115例,T1240例,L144例,L211例,L37例,L45例,L58例。椎体前缘压缩程度:1例完全压缩,10例压缩>4/5,23例>2/3,50例>1/2,36例>1/3。结果:120例中,2例合并后脱位下肢不全瘫在整复过程中神经功能无恢复或神经功能部分恢复后停止,改手术治疗。其余118例获得随访,随访时间2~6年,平均3年2个月。120例受伤压缩椎体前缘平均高度由整复前的50%恢复至整复后的90%。受伤椎体后凸角由平均13.25°恢复至平均-1.6°,14例合并脱位基本恢复。结论:持续腰椎悬吊牵引治疗胸腰椎骨折能获得满意的疗效,治疗方法简单,治疗后3、7、10d要根据X线片适当调整复位高度。

Abstract:
Objective:To discuss the clinical effectiveness in continues the lumbar vertebra suspension hauling treatment chest lumbar vertebra compressibility bonefracture. Methods: From 2001 to 2010, we studied the clinical effectiveness of continues the lumbar vertebra suspension hauling treatment chest lumbar vertebra compressibility bone fracture. The cases surveyed total 140,120 of which had full data. There were 47 males and 73 females with the age from 21 to 80 years(mean,55 years). Consulting time after injury from 30 min to 6 days. Fracture site in T11 had 5 cases,in T12 40 cases, in L1 44 cases, in L2 11 cases, in L3 7 cases, in L4 5 cases, in L5 8 cases. Compression degree of vertebral anterior border:full compression had 1 ease, more than 4/5 had 10 cases, more than 2/3 had 23 cases,more than 1/2 had 50 eases, in 1/3 had 36 eases. Results: Among them, 2 cases with legs paresis no recovery in nerval function or stopping recovery changed methods,and underwent surgical treatment. Others 118 cases were followed up from 2 to 6 years with an average of 3 years and 2 months. The height of the injured vertebral anterior borders had recovered from 50% before treatment to 90% after treatment. Kyphosis angle of the injured vertebral bodies had recovered from 13. 25° to-1.6° in average. 14 cases associated with dislocation basic reduction. Conclusion:Continues the lumbar vertebra suspension hauling treatment chest lumbar vertebra compressibility bone fracture can obtain satisfactory clinical effect , the method is easy. At 3,7,10 days after treatment, the height of bed shoule be adjusted according X-ray.
