头孢呋辛序贯疗法预防妇科术后感染疗效和成本-效果分析

目的：评价头孢呋辛序贯疗法预防妇科术后感染的临床疗效和经济效果。方法：311例妇科术后患者随机分为2组,序贯组（A组）155例：采用头孢呋辛0.75g～1.5g,iv gtt,bid,连用2d后改用头孢呋辛片剂500mg,po,bid,连用4～6d;对照组（B组）156例：采用头孢呋辛0.75g～1.5g,iv gtt,bid,连用5～7d,分别进行疗效及成本效果分析。结果：A、B两组手术切口的甲级愈合率分别为94.8%和96.8%,两组术后体温变化及不良反应无统计学差异;A组的成本效果比低于B组。结论：两组具有相似的临床疗效（P〉0.05）,但序贯疗法与单纯静脉输注治疗相比,显著降低了总医疗费用,具有药物经济学意义。

Efficacy and Cost-effectiveness of Sequential Therapy of Cefuroxime in the Prevention of Postoperative Infections in Gynecological Patients

OBJECTIVE：To evaluate efficacy and the cost-effectiveness of sequential therapy of cefuroxime in the prevention of postoperative infections in gynecological patients.METHODS：311 patients who have undergone gynecological surgery were randomly divided into 2 groups：155（sequential therapy group,group A） were assigned to receive cefuroxime 0.75g～1.5g by iv gtt b.i.d for 2 days followed by oral cefuroxime tablets 500mg b.i.d for 4～6 consecutive days,and 156（control group,group B） to receive single cefuroxime 0.75g～1.5g by iv gtt b.i.d for 5～7 consecutive days.Efficacy and the cost-effectiveness in 2 groups were analyzed respectively.RESULTS：The first-rate healing rates of incisions in group A and group B were 94.8% and 96.8%,respectively.The postoperative temperature change and the ADRs in two groups were of no statistical difference.The cost-effectiveness ratio in group A was lower than in group B.CONCLUSION：Similar therapeutic effect was noted in both groups（P〉0.05）,but the total medical cost was significantly lower in sequential therapy group than in control group,suggesting its pharmacoeconomic advantage.
