橡皮套胰液引流在胰十二指肠切除胰肠吻合中的应用体会

目的探讨解决胰十二指肠切除胰肠吻合中胰瘘的方法。方法通过对自1993～2000年我院应用橡皮套胰液引流的方式治疗21例胰十二脂肠切除的患者的临床研究。结果无1例发生胰瘘，无围手术期死亡。4例在拔除引流管时橡皮套拔除困难，经扩大引流窦道完成。1例胆囊床出血再次手术成功。2例胆瘘，保守治疗成功。1例切口裂开。2例肺炎。随访6个月-3a，临床上无腹揭等胰腺外分泌功能受影响的症状。结论橡皮套胰液引流在胰十二指肠切除胰肠吻合中操作简单，可有效地预防胰瘘的发生，具有很高的临床推广应用价值。

The Experience of applying condom for Pancreatic Juice Drainage during the Anastomosis of Pancreas and Intestines after Pancreatieoduodenectomy

Objective This passage is trying to ring out the way of preventing pancreatic fistula in the anastomosis of pancreas and intesfines. Methods It is bases on the clinical observation of 37 patieats who have had pancreatieoduodenectomy. Results N one of the patieats have pancreatic fistula and death in and after the operation. There are four cases in which there is some dfficulty in taking out the condom when the drainage sinus tract. One has gallbladder haemorrage after the operation and is cured in a second operation.Two have biliary fistula and is creed by conservative treatment. There is one whose incision part breaks and wto with pneumonis. There are no symptoms such as diarrhoes which are caused by pancreatic outer secrete funtion being influenced. Conclusion It is simpie to appiy condom for pancreatic juic duainage during the anastomosis of pareas and intestines after It can be used to take precautions against pancreatic fistula. So it can be widelv used in clinical practice.
