阻塞性睡眠呼吸暂停低通气综合症患者外周血Th1/Th2细胞因子表达水平的研究

摘    要:目的:检测阻塞性睡眠呼吸暂停低通气综合症(OSAHS)患者外周血CD4+T淋巴细胞分泌的细胞因子水平,探讨患者Th1/Th2相关细胞因子的平衡及其临床意义.方法:47例OSAHS患者按照呼吸暂停低通气指数(AHI)水平分为轻度组(n=16),中度组(n=17),重度组(n=14),18例健康人作为对照组,分别测定各组外周血中Thl细胞因子:干扰素γ (IFN-γ)、白细胞介素2 (IL-2)和肿瘤坏死因子β (TNF-β);Th2的细胞因子:IL-4、IL-10和IL-12的含量.结果:OSAHS各组Thl型细胞因子中IFN-γ高于对照组,随着病情的进展有逐渐升高趋势,但仅在重度组与中度组、轻度组、健康组比较时P均<0.05,其余Th1/Th2细胞因子在进行两两比较时各组间均无明显差异.OSAHS患者IFN-γ与AHI呈正相关,而Th2细胞因子与AHI无相关关系.结论:OSAHS患者Thl/Th2平衡失调,向Thl方向漂移,Th1细胞表型处于优势状态,且与OSAHS的严重程度相关.

Abstract:Objective: To detect the levels of Th1 and Th2 type cytokines of peripheral blood CD4^+T lymphocytes in patients with obstructive sleep apnea hypopnea syndrome(OSAHS)and explore their clinical significance. Methods:Based on apnea hypopnea index(AHI) which measured by polysomnography (PSG),47 patients with OSAHS were divided into three groups: mild group(n=16),moderate group(n=17) and severe group and healthy people(n=18) as control group. The Th1 eytokines(IFN-γ, IL-2,TNF-β) and Th2 cytokines(IL-4,IL-10,IL-12) were measured. Results: The levels of Th1 type cytokines such as IFN-γ in patients with OSAHS were significantly higher than that in the control group,P〈0.05. The levels of Th1 positively correlated to AHI,P〈0.05.There were no significant difference of the other Th1/Th2 related cytokines (IL-2,TNF-β, IL-4, IL-10 and IL-12) between OSAHS and the control group. Conclusion: Th1/Th2 imbalances in OSAHS. IFN-γ is enhanced and Th1/Th2 shifts to Th1. It may be implicated in the pathogenesis of this disorder, and which is associated with the severity of OSAHS.
