不同尿白蛋白排泄量的2型糖尿病患者血清铁蛋白的变化及意义

目的：观察不同尿白蛋白排泄量的2型糖尿病患者血清铁蛋白的变化并探讨其意义。方法：选择2型糖尿病组59例及正常对照组20例，根据24h尿白蛋白排泄量将糖尿病组分为正常白蛋白尿组（NA组）20例、微量白蛋白尿组（MA组）19例与临床蛋白尿组（PR组）20例。应用ELISA法测定各组的血清铁蛋白水平。结朵：2型糖尿病患者血清铁蛋白水平高于正常对照组，临床蛋白尿组患者的血清铁蛋白水平高于微量白蛋白尿组、正常白蛋白尿组；微量白蛋白尿组患者的血清铁蛋白水平高于正常白蛋白尿组，差异均有统计学意义（P〈0．05）；血清铁蛋白水平与TC、LDL、FBG呈正相关，差异有统计学意义（P〈0．01）。结论：血清铁蛋白在2型糖尿病患者中升高，并且随尿蛋白排泄率的增加而逐渐升高，铁蛋白在糖尿病肾病的发生中起一定作用。

Change and significance of serum ferritin in patients of type 2 diabetes mellitus with different urine albumin excretion

Objective： To observe the change of serum ferritin in patients of type 2 diabetes mellitus with different urine albumin excretion. Methods： 59 patients with type 2 diabetes mellitus and 20 healthy control subjects were selected in this study.Patients with diabetes mellitus were subdivided into three groups according to 24 h urine albumin excretion（UAE） ： normal albuminuria group （NA）, micromalbumin- uria group （MA）, and clinical albuminuria group （PR）.Measurement of serum ferritin was performed by ELISA. Results： The serum ferritin levels of type 2 diabetic groups were higher than those of control group. The serum ferritin levels of MA and PR groups were higher than those of NA group, and that of PR group was higher than that of MA group （P〈0.05）. Correlation analysis demonstrated that serum ferritin was posi- tively correlated with TC,FBG,LDL （P〈0.01）. Conclusion：Serum ferritin level significantly increases in type 2 diabetic patients, and the magnitude of ferritin is related to the severity of diabetic nephropathy. The increase of serum ferritin may involve in the development of diabetic nephropathy.
