经尿道膀胱肿瘤电切术治疗浅表性膀胱肿瘤

摘    要:目的探讨经尿道膀胱肿瘤电切术(TuRBT)治疗浅表性膀胱肿瘤的治疗效果。方法分析经尿道膀胱肿瘤电切术治疗28例浅表性膀胱肿瘤患者的临床随访资料,随访时间为2~38个月,平均24.6个月。结果28例患者均一次手术成功,手术平均时间17min(5~30min),无膀胱穿孔及术后继发出血等并发症。术后留置导尿管3—7d,术后定期使用丝裂霉素等药物膀胱内灌注化疗,术后随访平均24.6个月,4例复发(14.3%),肿瘤复发后再次行TURBT。结论经尿道膀胱肿瘤电切术治疗浅表性膀胱肿瘤具有操作简单、损伤小、恢复快、可多次重复及安全性高等优点,是治疗浅表性膀胱肿瘤的有效方法。

Abstract:Objective To evaluate the efficacy of transurethral resection of bladder tumor(TURBT) for treating superficial bladder tumor. Methods To analyze the clinical date of 28 case of superficial bladder tumor patients treated by TURBT,28 cases were followed-up for 2 to 38 months with a mean period of 24.6 months. Results All cases of superficial bladder tumor could tolerate the surgery,the mean operation time was 17 minutes(5 - 30minutes). Neither intraoperative bladder perforation nor secondary hemorrhage occurred. Indwelling urethral catheter time ranged from 3 - 5days. Postoperatively, drugs such as mitomycin were used through intravesical instillation for chemotherapy. All patients have been followed up for a mean of 24.6 months,4 patients( 14.3%)had out-of-field recurrence. The case treated by TURBT again after tumor recurrence. Conclusion TURBT has advantages such as simple surgical manipulation, les lesion, faster recovery, repeatedly performable process and safety. It is an ideal therapy for treating superficial bladder tumor.
