后路综合手术治疗腰椎退变性滑脱并椎管狭窄

目的 探讨后路综合手术治疗腰椎退变性滑脱并椎管狭窄(DLSSS)的手术方法和疗效。方法 对47例均采取后路充分减压,植骨融合及经椎弓根复位固定综合性手术治疗。平均随访32个月,进行临床功能和影像学评价。结果 临床功能评价优良率87％(41/47);椎体滑移复位率96％(45/47);退变畸形矫正率91％(43/47);植骨融合率96％(45/47)。内固定松动率4％(2/47),一过性腰腿痛发生率34％(16/47)。结论 应用上述综合手术治疗DLSSS可同时解除神经卡压,矫正畸形和稳定脊柱,从而能够达到多症兼治的治疗目的。

Combined surgical treatment of posterior approach for degenerative lumbar spondylolisthesis with spinal canal stenosis

Objective To study the method and therapeutic efficacy of the combined surgical treatment of posterior approach for degenerative lumbar spondylolisthesis with spinal stenosis(DLSSS) . Methods From Jan-uary 1996 to June 2001, the combined surgical treatment of posterior approach that include decompression of posleior approach, bone graft and fusion, and Via pedicle vertebral arch reduction and fixation were performed for treating of 47 cases with DLSSS. Average follow up time was 32 months, the clinical function and imaging were e-valuated postoperatively. Results The excellent rate of clinical function was 87% (41/47). The rate of succeful reduction of slipping vertelrral body was 96% (45/47) correct rate of bone deformity was 91% (43/47). The rate of fusion was 96% (45/47) . The rate of loose internai fixation was 4% (2/47) . The rate of transient nervous root symptoms was 34% (16/47) . Conclusion Using combined surgical treatneat for DLSSS, it can relieve nervous compression, correct spinal deformity and fix destabilzed spine, so that can achieve goals of the several disorders curecl at the same time.
