急性胰腺炎早期IL-6的动态变化及其临床意义

目的:探讨急性胰腺炎早期IL-6变化及与疾病严重程度相关性。方法:选取2008年2月-2013年2月本院住院的101例AP患者,根据病情程度将其分为轻症组43例,中重症组31例,重症组27例,同时选取30例健康查体者作为对照组。应用双抗体夹心免疫发光法测定患者第1、3、7、10天血清中IL-6含量,比较三组患者之间IL-6含量变化。结果:在组间比较中,轻症患者d1、d3及d7血清IL-6含量明显高于对照组,差异均有统计学意义(P0.05),d10血清IL-6含量虽比对照组高,但差异无统计学意义(P0.05);中重症及重症患者d1、d3、d7及d10血清IL-6含量明显高于对照组,中重症患者四次测量IL-6含量水平明显高于轻症患者,重症患者四次测量IL-6含量水平明显高于中重型及轻型,比较差异均有统计学意义(P0.05)。在组内比较中,轻症组及中重症组患者组内d1、d3、d7及d10血清IL-6含量比较差异均有统计学意义(P0.05);重症患者d1与d3、d7与d10差异有统计学意义(P0.05),而d3及d7差异无统计学意义(P0.05)。结论:血清IL-6含量动态变化可以作为胰腺炎病情发展的监控指标。

Early Dynamic Changes of IL-6 in Acute Pancreatitis and Its Clinical Significance

Objective：To investigate the changes of IL-6 in early stage of acute pancreatitis and its correlation with disease severity.Method：One hundred and one patients with acute pancreatitis in our hospital from February 2008 to February 2013 were selected,according to the severity of disease they were divided into the mild group for 43 cases,the moderate group for 31 cases,the severe group for 27 cases,at the same time 30 healthy people were selected as the control group.IL-6 in serum was detected by double antibody sandwich chemiluminescent immunoassay in the first,third,seventh, tenth day,changes in the content of IL-6 in three groups were compared.Result：In the comparison of groups,the content of serum IL-6 of mild patients in d1,d3 and d7 were significantly higher than those in the control group,the differences were statistically significant（P〈0.05）,the serum IL-6 content of d10 was higher than that of the control group,but there was no statistically significant difference（P〉0.05）.The serum IL-6 content of moderate and severe patients in d1,d3,d7 and d10 were significantly higher than that of the control group,in moderate patients with four times of measurement of IL-6 content level was significantly higher than the mild patients,severe patients with four times of measurement of IL-6 content was significantly higher than that of moderate and mild patients,the differences were statistically significant（P〈0.05）. Within the group comparison,the content of serum IL-6 of the mild and moderate patients in d1,d3,d7 and d10,the differences were statistically significant（P〈0.05）,the difference of severe patients was statistically significant in d1 and d3,d7 and d10（P〈0.05）,no significant differences in d3 and d7（P〉0.05）.Conclusion：Serum IL-6 content changes can be used as a monitoring index of the development of pancreatitis.
