| 摘 要： | 目的:探讨Arnold-Chiari畸形合并脊髓空洞症改良手术治疗方法及效果。方法:回顾分析42例Arnold-Chiari畸形合并脊髓空洞症实施改良手术治疗的临床资料。结果:42例患者中症状改善39例,有效率92.9%;所有患者小脑扁桃体下疝均纠正,脊髓空洞消失12例,缩小28例,无明显改善2例,有效率95.2%。结论:枕颈减压,松解后还纳下疝的小脑扁桃体于减压扩大的后颅窝腔,改善枕骨大孔区脑脊液(CSF)循环通路,扩大硬脑膜修补,重建适宜的后颅窝容积,恢复其正常解剖结构,这是Arnold-Chiari畸形并脊髓空洞安全显效的手术治疗方式。
