PCI术后对比剂肾病的相关危险因素研究

目的：研究冠心病患者经皮冠状动脉介入治疗（PCI）术后对比剂肾病（CIN）发生率及高危因素。方法：回顾性分析300例行PCI的冠心病患者肾功能情况、年龄、心功能、糖尿病、高血压、冠脉血管病变等与CIN的相关性,并记录CIN的发生情况。结果：CIN的发生率为14%,对CIN相关的临床因素进行Logistic回归分析发现,高龄、基础肌酐值〉309.4μmol/L、内生肌酐清除率〈45 mL/min、多支冠状动脉病变、心功能不全等与CIN发生密切相关（P〈0.05）。而高血压和单纯糖尿病与CIN发生未见相关（P〉0.05）。结论：高龄、肾功能不全、心功能不全、多支病变等可能是CIN发生的高危因素。高血压和单纯糖尿病未能证实是CIN发生的危险因素。

The study of risk factors of contrast-induced nephropathy in patients after PCI

Objective: To study the incidence of CIN and the high risk factors of CIN after PCI in patients with coronary heart diseases.Methods:With a retrospective analysis of the patients who accepted PCI,300 patients were selected to analyze the relevance between renal function,age,heart function,diabetes,hypertension,coronary artery disease and CIN,and the incidence of CIN were record.Results: Clinical factors associated CIN Logistic regression analysis showed that old age,creatinine>309.4μmol/L,endogenous creatinine clearance rate <45 mL/min,coronary multi-vessel disease,heart dysfunction were risk factors for CIN(P<0.05).And diabetes and hypertension were not risk factors for CIN(P>0.05).Conclusion:The elderly,renal insufficiency,heart failure,multi-vessel disease may be the high-risk factors for CIN.There are no definite evidences to verify that hypertension and diabetes are risk factors for CIN.
