儿童期垂体腺瘤的治疗特点(附22例报道)

目的 总结22例儿童垂体腺瘤的治疗经验,探讨儿童垂体腺瘤的治疗特点.方法 回顾分析1997年11月至2006年7月我院收治的22例14岁以下垂体腺瘤患儿的临床资料,其中男14例(其中1例行分期手术),女8例,年龄1～14岁,平均12.3岁(1例为13个月垂体腺癌患儿,其余均为8岁以上).12例经蝶窦入路显微手术,9例经额垂体瘤切除,1例分期分别经左、右额行垂体瘤切除.随访1～10年.结果 22例中,肿瘤全切13例.近全切除9例,大部切除1例,无手术死亡.未出现严重并发症.结论 经蝶显微手术是儿童垂体腺瘤最佳治疗方法,术前及术后正规的内分泌观察以及治疗必不可少.术后患儿的心理治疗要引起神经外科医师的重视.手术后放疗要谨慎.

Treatment of pituitary adenoma children (report of 22 cases)

Objective To sum up the experience in the treatment of pituitary adenoma in children. Methods From November 1997 to July 2006, 22 children suffered from pituitary tumor were operated in department of neurosurgery, Tiantan hospital. There were 14 boys and 8 girls. Their age ranged from 13 months (this girl's pathology diagnoses is pituitary carcinoma) to 14 years old with an average of 12. 3 years old. Of this group, 12 patients by transsphenoidal approach, 9 by frontal approach, one by bilateral frontal approach and operated by stages. All patients were followed - up from 1 to 10 years by imaging studies and endocrinologic investigations and psychologic observation. Results No mortality was noted. Tumors were completely removed in 13 cases and subtotally in 9 cases, most partly in one case. There was no serious postoperative complication. Conclusions Transsphenoidal microsurgery is the preferred treatment in children with pituitary adenoma. Endocrinologic investigation is very important to children. Psychologic observation and treatment should be noticed. Radiotherapy is not necessary to children.
