妊娠合并急性阑尾炎70例的诊治与妊娠结局

目的探讨妊娠合并急性阑尾炎的临床特点及治疗原则与妊娠结局。方法对我院近5年来收治的70例妊娠合并急性阑尾炎患者的临床资料进行回顾性分析。结果70例患者中有42例阑尾炎症状典型行阑尾切除术,手术一次成功,术后病理均诊断为急性阑尾炎,流产率2.38%。28例阑尾炎症状较轻行保守治疗,成功率82.14%,自然流产率7.14%。结论妊娠期阑尾炎强调早期诊断,及时抗炎保胎治疗,建议手术为主的治疗方式。

The diagnosis and treatment of appendicitis in pregnancy and the gestation final outcome:70 cases

Objective To discuss the diagnosis and treatment of acute appendicitis in pregnancy and the gestation final outcome. Methods 70 young female patients presented with appendicitis during January 2001 to December 2006 in Shenzhen Hospital of Peking University. Result 42 patients underwent emergency appendectomy successfully,100% had pathologically proven acute appendicitis, the postoperative spontaneous abortion rate was 2.38%.28 patients treated with conservative therapy, 82.14% had complete relief of their initial complaints, with only the abortion rate was 7.14%. Conclusion The accurate diagnosis of appendicitis during pregnancy requires a high level of suspicion and clinical skills,and relying on medical treatment. It is important to undertake exploratory surgery early.
