护理干预对慢性心力衰竭患者心理状态及预后的影响

目的 探讨护理干预对慢性心力衰竭患者心理状态及预后的影响.方法 选择2008年4月至2009年4月住院治疗的慢性心力衰竭患者80例作为研究对象.随机分为对照组和观察组各40例,对照组患者进行常规护理,观察组患者在治疗过程中实施护理干预.比较两组患者总有效率及治疗前后的焦虑、抑郁评分改善情况,出院后均随访1年,比较两组患者复发率.结果 两组患者治疗前后的焦虑、抑郁评分均有明显改善,但观察组患者改善情况明显优于对照组患者,两组比较差异有统计学意义(P＜0.05)；观察组显效24例,有效14例,无效2例,总有效率92.5％;对照组显效19例,有效11例,无效10例,总有效率75.0％,观察组总有效率明显高于对照组(P＜0.05).出院后随访1年,观察组复发率为12.5％,对照组复发率为43.5％,观察组明显低于对照组(P＜0.05).结论 护理干预能有效改善慢性心力衰竭患者的焦虑、抑郁评分,增强患者战胜疾病的信心,并能积极配合医护人员的治疗和护理,提高治疗总有效率,同时降低远期预后的复发率.

Effect of nursing intervention on psychological status and prognosis of patients with chronic heart failure

Objective To investigate the effect of nursing intervention on the psychological status and prognosis of patients with chronic heart failure.Methods 80 patients with chronic heart failure from April 2008 to April 2009 were chosen as the research subjects and randomly divided into the control group and the observation group.The control group was given with routine care.The observation group took the nursing intervention during treatment course.The total effective rate,the anxiety before and after treatment,and the depression score improvement were compared between the two groups.The relapse rates were compared by 1 year followed up after discharge.Results The anxiety and depression scores in the two groups were significantly improved after treatment.But the improvement in the observation group was significantly better than the that in the control group with statistical difference(P<0.05).There were 24 cases of excellent effect,14 cases of effect and 2 cases of ineffec in the observation group with the total effective rate of 92.5%.There were 19 cases of excellent effect,11 cases of effect and 10 cases of ineffect in the control group with the total effective rate of 75.0%.The total effective rate in the observation group was significantly higher than that in the control group(P<0.05).The recurrence rate for 1 year followed up after discharge was 12.5% in the observation group and 43.5% in the control group,showing that the observation group was lower than the control group(P<0.05).Conclusion Nursing intervention can improve the anxiety and depression scores for the patients with chronic heart failure,enhance the confidence of the patients to overcome the disease,make them actively cooperate with medical treatment and nursing care to increase the total effective rate and reduce the recurrence rate of long-term prognosis.
