伴腹型肥胖的2型糖尿病患者的临床特点分析

目的:探讨伴腹型肥胖的2型糖尿病(T2DM)患者的临床特点。方法:选取T2DM住院患者171例,按腰围分为腹型肥胖组(AO组,男性>90 cm,女性>85 cm)和非腹型肥胖组(NAO组,男性≤90 cm,女≤85 cm),比较各组的糖代谢、脂代谢、新稳态模型(HOMA2)胰岛B细胞分泌指数(HOMA2-B)和胰岛素敏感性指数(HOMA2-S)。结果:AO组血压、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、C反应蛋白(CRP)、血糖和C肽(CP0、CP30、CP120)均较NAO组显著升高,而高密度脂蛋白胆固醇(HDL-C)、HOMA2-S则明显降低。结论:伴腹型肥胖的T2DM患者多伴TG水平升高,且血糖失控和胰岛素抵抗程度更重。

Clinical characteristics of type 2 diabetes mellitus patients with abdominal obesity

Objective To analyze the clinical characteristics of type 2 diabetes mellitus patients with abdominal obesity.Methods One hundred and seventy one patients with type 2 diabetes mellitus were divided by waist circumstance into abdominal obesity group（male90 cm,female85 cm） and non-abdominal obesity group（male≤90 cm,female≤85 cm）.Differences in glucose and lipid metabolism,insulin secretion index（HOMA2-B） and insulin sensitivity index（HOMA2-S） between these two groups were analyzed.Results Blood pressure,triglycerides,low-density lipoprotein cholesterol,C-reactive protein（CRP）,blood glucose and C-peptide（CP0,CP30 and CP120） in abdominal obesity group were significantly higher than those in non-abdominal obesity group,and high-density lipoprotein cholesterol and HOMA2-S were significantly lower.Conclusions Type 2 diabetes mellitus patients with abdominal obesity had elevated triglycerides,poorly-controlled blood glucose and more severe insulin resistance.
