摘    要:目的研究中性粒细胞明胶酶相关脂质运载蛋白(NGAL)和肾损伤分子-1(KIM-1)在内毒素(LPS)致大鼠急性肾损伤(AKI)早期的表达变化。方法健康雄性Wistar大鼠32只随机分为4组。正常对照组(NC):经尾静脉注射1ml生理盐水;LPS2、4、6h组:LPS剂量为5mg/kg体重,生理盐水稀释至1ml尾静脉注射,复制AKI模型。造模后在各时间点,ELISA法测大鼠血清和尿液中NGAL的含量及尿液中KIM-1的含量,除蛋白法测定血肌酐(SCr)值并比较各组大鼠光镜下肾脏的病理改变。结果与NC组Scr比较:LPS2h组略升高(P〉0.05),LPS4、6h组显著升高(P〈0.01);LPS2、4、6h组血清和尿液中NGAL及尿液中KIM-1的水平均明显高于NC组(P〈0.01),与LPS2h组相比,LPS4h组血清和尿液中NGAL升高幅度小(P〉0.05),而尿液中KIM-1显著性升高(P〈0.01);光镜下,NC组肾组织结构正常,LPS2、4、6h组肾组织出现不同程度的病理改变。结论 NGAL和KIM-1在AKI早期即出现升高,可作为AKI的早期诊断指标,并且KIM-1有明显的时间依赖性,可作为病情监测指标。
