幽门螺杆菌的培养及药物敏感试验分析

目的分析温州地区幽门螺杆菌(Hp)临床分离株对多种常用抗幽门螺杆菌抗生素的药敏和耐药情况。方法收集我院2289例胃病患者的标本,取其胃黏膜活组织进行幽门螺杆菌培养,对培养到的Hp菌采用抗生素药敏试验纸片琼脂扩散法作药物敏感试验。结果Hp培养阳性率依次为消化性溃疡50.00%,活动性胃炎40.77%,胃癌31.87%,萎缩性胃炎17.92%。898例Hp阳性患者的药物敏感试验结果表明左氧氟沙星的耐药率最低,暂未发现耐药株,其次为庆大霉素(1.11%)、克拉霉素(13.14%)、羟氨苄青霉素(13.36%)、呋喃唑酮(32.29%),甲硝唑的耐药率最高(100%)。结论Hp感染与多种胃病关系密切,临床上对Hp感染的患者可选择左氧氟沙星、庆大霉素、克拉霉素、呋喃唑酮及羟氨苄青霉素治疗,不宜选用甲硝唑。

Study on Cultivation and drug sensitivity test of Helicobacter pylori

Objective To analyze the antibiotic resistance of clinical isolates of Helicobacter pylori (Hp) in this area.Methods Hp strains were isolated from 2289 biopsy samples of gastric mucosae. The drug sensitivity test of Hp strains was examined by agar diluted method.Results 898 Hp strains were isolated from 2289 samples. The positive rates of Hp in peptic ulcer, active gastritis, gastric cancer and atrophic gastritis were 50.00%, 40.77,%, 31.87% and 17.92% respectively. The incidences of Hp isolates resistant to levofloxacin, gentamycin, clarithromycin, amoxicillin, furazolidone and metronidazole were 0%, 1.11%, 13.14%, 13.36%, 32.29% and 100% respectively.Conclusion Hp infection is closely related to many kinds of gastric diseases. The infection of Hp in these patients can be cured by treating with levofloxacin, gentamycin, clarithromycin, furazolidone and amoxicillin, but not by metronidazole.
