自发性不全流产时阴道米索前列醇给药与人流清宫术的疗效比较

目的评估使用米索前列醇阴道片与人流清宫术对自发性不全流产的疗效和安全性。方法采用前瞻性随机研究的方法对确诊为不完全流产的150例妇女进行疗效比较，其中米索前列醇阴道片组与人流清宫术组各75例，随访24小时，比较两种方法的疗效、失血量和副作用。结果治疗24小时后，米索前列醇的完全流产率为91％，人流清宫术为95％，差异无统计学意义（χ2=0．22，P〉0．05）。失血量（t=5．08，P〉0．05）、疼痛（χ2=0．90，P〉0．05）两组相比差异无统计学意义，米索前列醇阴道片组发热的发生率较人流清宫术组高，差异有统计学意义（χ2=4．69，P〈0．05）。结论米索前列醇阴道片对自发性流产的疗效与人流清宫术相似，是一个安全、有效、能替代人流清宫术的方法。

Efficacy comparison of misoprostol vaginal tablet and surgical curettage in spontaneous incomplete abortion

Objective To evaluate the efficacy and safety of misoprostol vaginal tablet and surgical curettage in spontaneous incomplete abortion. Methods A prospective randomized study was conducted on 150 defined incomplete abortion women, 75 cases in group of misoprostol vaginal tablet and 75 cases in group of surgical curettage. They were followed for 24 hours to compare the efficacy, blood loss and side effects of two methods. Results Twenty-four hours after treatment, the complete abortion rate of the group of misoprostol vaginal tablet and the group of surgical curettage was 91% and 95%, respectively, and the difference was not significant （X2 = 0.22, P 〉 0.05 ）. Blood loss （t = 5.08 ,P 〉 0. 05 ） and pain （χ2 = 0.90 ,P 〉 0.05 ）were not significantly different in two groups, but the incidence of fever in the group of misoprostol vaginal tablet was significantly higher than that of the other group （χ2 = 4.69, P 〈 0. 05 ）. Conclusion The efficacy of misoprostol vaginal tablet is similar to that of surgical curettage on spontaneous abortion, and misoprostol vaginal tablet is a safe and effective ahernative approach to surgical curettage.
