括约肌间瘘管结扎术治疗高位单纯性肛瘘临床研究

目的评价括约肌间瘘管结扎术治疗高位单纯性肛瘘的临床疗效。方法将80例高位单纯性肛瘘患者随机分为两组，每组40例。试验组采用括约肌间瘘管结扎术，对照组采用传统的肛瘘切除术。观察两组临床疗效，以及术后第1、3、7、14天创面疼痛、渗液、出血情况，并记录创面愈合时间、瘢痕长度、控便能力。结果①试验组总有效率为95．0％，对照组为97．5％，组间临床疗效差异无统计学意义（P〉0．05）。②试验组第1天、第3天术后创面疼痛积分，各观察时点创面渗液积分，以及第1天、第3天、第7天术后创面出血积分少于对照组（P〈0．05）。⑧试验组创面愈合时间短于对照组（P〈0．05），控便能力优于对照组（P〈0．05），瘢痕长度小于对照组（P〈0．05）。结论与传统的肛瘘切除术比较，括约肌问瘘管结扎术用于治疗高位单纯性肛瘘，患者术后创面疼痛、渗液及出血情况轻，创面愈合时间短，创伤小。

Clinical research on ligation of inter-sphincteric fistula tract for the treatment of high simple anal fistula

Objective To evaluate the clinical curative effect of ligation of inter-sphincteric fistula（LIFT） in treating high simple anal fistula. Methods Eighty patients with high simple anal fistula were randomized into two groups： experimental group in which 40 cases were treated by LIFT, and control group in which 40 cases were treated by traditional anal fistula resection. The clinical efficacy, wound pain, exudates and bleeding were measured on the 1 st, 3tb, 7th and 14th day after operation. The wound healing time, the length of scar and the continence function were recorded. Results （~） The overall effective rate was 95.0% in the experimental group and 97.5% in the control group, without significant difference between the two groups（P 〉 O. 05 ）. （~） The scores of wound pain and exudates at the 1st, 3th day and the bleeding score at the 1st, 3th, 7th day after operation in experimental group were less than that of the control group （ P 〈 0.05 ）. （3） The wound healing time and the length of scar in experimental group was less than the control group（ P 〈 0.05 ） , and the continence function was better than the control group（P 〈0.05）. Conclusion Compared with traditional anal fistula resection, LIFT can alleviate wound pain, exudates and bleeding after operation with less wound healing time and trauma.
