外剥内扎术联合内括约肌侧切治疗环状混合痔40例

目的探讨外剥内扎术联合内括约肌侧切治疗环状混合痔的临床疗效。方法将80例环状混合痔患者随机分为观察组和对照组,每组40例;对照组予外剥内扎术,观察组予外剥内扎术联合内括约肌侧切术;观察两组临床疗效、创面愈合时间及肛门疼痛情况,术后3个月随访肛管最大收缩压、肛管静息压以及肛门失禁、狭窄情况。结果两组临床疗效及创面愈合时间差异均无统计学意义（P〉0.05）;观察组术后1天、术后2天、术后3天肛门疼痛评分低于对照组（P〈0.05）,术后3个月时的肛管静息压低于对照组（P〈0.05）。结论外剥内扎术联合内括约肌侧切治疗环状混合痔较单纯应用外剥内扎术,可以减轻术后疼痛,降低肛管静息压。

Piercing stripping outside and pricking inside plastic operation combined with internal sphincter lateral incision for circular mixed hemorrhoids： A report of 40 cases

Objective To observe the clinical efficacy of piercing stripping outside and pricking inside plastic operation combined internal sphincter lateral incision in treating circular mixed hemorrhoids. Methods Eighty cases were randomized into observation group and control group, 40 cases in each group; control group was treated by piercing stripping outside and pricking inside plastic operation, and observation group was added internal sphincter lateral incision; the clinical efficacy, wound healing time and anus pain were observed. Three months after operation, the follow-up visit to check the anal maximal contraction pressure, anal canal resting pressure, fecal incontinence and anal stenosis. Results No significant difference was found in clinical efficacy and wound healing time between observation group and control group （P 〉0.05 ） ; the anal pain scores at the 1 st, 2,d, 3rd days after operation in observation group were lower than that of the control group （P 〈 0.05 ） , and the anal canal resting pressure at the 3rd month after operation was lower than that of the control group （ P 〈 0. 05 ）. Conclusion Combined piercing stripping outside and pricking inside plastic operation and internal sphincter lateral incision is better than piercing stripping outside and pricking inside plastic operation only in the treatment of circular mixed hemorrhoids. It can alleviate the pain and reduce the anal canal resting pressure.
