DNA微阵列芯片法检测临床痰标本中结核分枝杆菌耐多药突变基因的研究

摘    要:目的DNA微阵列芯片法(博奥)检测临床痰标本中耐多药结核分枝杆菌相关基因突变,评价诊断耐多药结核病(MDR—TB)的应用价值。方法对涂阳肺结核患者的痰标本进行DNA微阵列芯片法检测,与金标准传统比例法药物敏感性试验比较,获得DNA微阵列芯片法的检测灵敏度、特异度和一致率。结果以传统药敏作金标准,DNA微阵列芯片法检测553例痰标本,利福平、异烟肼和MDR检测的灵敏度、特异度和一致率分别为92.73%、98.19%和97.65,81.82%、93.43%和92.04,82.35%、97.30%和96.38。结论DNA微阵列芯片法耐多药检测技术具有较高的灵敏度和特异性,适用于肺结核病人的耐多药结核病人诊断。

Abstract:Objective To evaluate the efficiency of diagnosis muhidrug resistant Mycobacterium tuberculosis using a Biochip system for determining mutations in clinic sputum sample. Methods With conventional drug sensitivity test as the standard, sputa from smear - positive pulmonary tuberculosis patients were collected for detection by a Biochip system to obtain the sensitivity and specificity and accordance of the method. Results For 553 sputa samples, the sensitivity and specificity and accordance of Biochip system for RIF and INH and muhidrug resistance(MDR) were 92. 73% ,98.19% , 97. 65 and 81.82%, 93.43%, 92. 04 and 82. 35%, 97.30%, 96. 38, respectively. Conclusion The Biochip system shows better sensitivity and specificity, it is available for rapid determination of multidrug - resistant pulmonary tuberculosis.
