腔镜下内环口结扎加脐正中襞覆盖治疗儿童腹股沟疝

目的探讨腹腔镜内环口结扎加脐正中襞覆盖在儿童腹股沟疝手术中的应用价值。方法搜集140例腹股沟斜疝患儿，将其随机分为两组：60例采用腹腔镜内环口高位结扎加脐正中襞覆盖疝环口治疗的患儿为腹腔镜组，80例采用传统开放手术治疗的患儿为传统手术组；比较两组手术后恢复情况及随访结果。结果腹腔镜组的手术时间为（20．8±6．5）min，传统手术组的为（33．5±10．6）min，两组比较差异有显著性（P〈0．01）；术后住院时间腹腔镜组为（2．4±1．5）d，传统手术组为（4．2±1．3）d，两组间差异有显著性（P〈0．01）；腹腔镜组阴囊水肿、疝复发、异时疝发生率均明显少于传统手术组（P〈0．05）。结论腹腔镜疝囊高位结扎加脐正中襞内环口覆盖加强腹壁治疗小儿腹股沟疝安全可行，恢复快，不易复发。

Laparoscopic high ligation of hernia sac plus median umbilical fold cover for childhood inguinal hernia

Objective To explore the value of laparoscopic high ligation of hernia sac plus median umbilical fold cover in the treatment of childhood inguinal hernia. Methods 140 pediatric patients with inguinal hernia were randomly assigned to receive laparoscopic high ligation of hernia sac and median umbilical fold cover（ 60 children, laparoscopy group ）, or conventional open surgery （ 80 children,conventional surgery group ）. The postoperative status and results of follow-up were compared between the two groups. Results There were significant differences in surgical duration and length of hospital stay between laparoseopy group and conventional surgery group （ 20.8 ± 6.5]min vs. 33.5 ± 10.6]min, P〈 0.01; （ 2.4 ± 1.5 ）d vs. （ 4.1.3 ）d, P〈 0.01 ）. The incidence rates of scrotal edema, hernia recurrence, and metachronous hernia were significantly lower in laparoscopy group than in conventional surgery group （ P 〈 0.05 ）. Conclusions Laparoscopic high ligation of hernia sac plus median umbilical fold cover is safe and feasible in the treatment of childhood inguinal hernia.
