瑞芬太尼用于腹部手术术后镇痛的临床观察

目的研究瑞芬太尼用于腹部手术术后镇痛的临床效果。方法48例ASA分级Ⅰ～Ⅱ级全麻腹部手术患者，随机等分为两组。F组：芬太尼1～1．5mg＋氟哌利多5mg＋0．9％生理盐水稀释至100ml；R组：20μg瑞芬太尼／kg＋氟哌利多5mg＋0．9％生理盐水稀释至100ml，按2．0ml／h速度持续输注48小时。比较两组镇痛效果，以及恶心、呕吐、嗜睡、皮肤瘙痒、呼吸抑制等并发症的发生率。结果两组病人术后48h镇痛效果均满意，但R组术后恶心、呕吐、嗜睡发生率，由于观察例数少，未显示低于F组。结论瑞芬太尼用于腹部手术术后镇痛安全有效。

Clinical observation of remifentanil in patient controlled analgesia after abdominal operation

Objective To investigate clinical effects of remifentanil in patient controlled analgesia （PCA） after abdominal operation. Methods Forty-eight patients（ASA Ⅰ-Ⅱ ）undergoing abdominal operation were divided into two groups： Group F and Group R. In Group F, 1-1.5mg fentanil and 5mg droperidol were diluted to 100 ml of normal saline. In Group R, 20μg/kg remifentanil and 5mg droperidol were diluted to 100ml too. Patients in both groups were continual infused for 48h at the velocity of 2ml/h. The effect of analgesia, incidences of complications such as nausea, vomiting, somnolence, itch of skin, respiratory depression and retention of urine were compared. Results Patients in both groups had content analgesia. There was no significant difference in visual analogue score and the incidence of complications between the two groups. Conclusion Remifentanil is effective and safe for PCA after abdominal operation.
