全身炎症反应综合征对急性胰腺炎预后的影响

目的：探讨全身炎症反应综合征(Systemic Inflammalory Response Syndrome，SIRS)对急性胰腺炎患者预后的影响。方法：选择急性胰腺炎患者280例．评价每例患者SIRS阳性指标符合数，将患者分为非SIRS组和SIRS组，SIRS组可根据阳性指标符合数分为SIRS2、SIRS3和SIRS4三组，另根据是否伴有感染分为不伴感染和伴感染两组；预后情况以病死率作为指标。结果：本研究急性胰腺炎SIRS的发生率为48．6％；SIRS组的病死率(8．09％)显著高于非SIRS组(1．39％)；重症胰腺炎患者SIRS的发生率(81．8％)和病死率(30．3％)显著高于轻症患者(44．1％、1．21％)；急性胰腺炎患者SIRS伴感染者的病死率高于不伴感染者。结论：急性胰腺炎患者SIRS的发生以及有无感染与预后有关。

Impacts of Systemic Imflammatory Response Syndrome on the Prognosis of Patients with Acute Pancreatitis

Objective:To investigate impacts of Systemic Inflammatory Response Syndrome (SIRS)on the prognosis of patients with acute pancreatitis.Methods:280 patients with acute pancreatitis between 1997 and 2000 were divided into non SIRS and SIRS groups.Furthermore,SIRS group was categorized into three subgroups according to the numbers of SIRS criteria and two subgroups with infection or without infection.The prognosis was evaluated by mortality.Results:Morbidity of SIRS in this group was 48.6%.Mortality of SIRS group(8.09%) was significantly higher than that of non SIRS group(1.39%), morbidity of SIRS(81.8%)and mortality(30.3%)of patients with acute severe pancreatitis were remarkably higher than those of simple ones(44.1% and 1.21%,respectively).Mortalities of group with infection were higher than that of group without infection.Conclusions:There is a close relation between prognosis and morbidity of SIRS and occurrence of infection in patients with acute pancreatitis.
