激光动力学治疗支气管肺癌的临床分析

目的探讨激光动力学治疗(PDT)支气管肺癌的临床疗效.方法应用德国产光敏剂photosan-3,2 mg/kg静注后48 h在内镜下用德国产CERALAS PDT 630 Laser 200 J/cm照射病变部位,必要时72～96 h内再重复1至2次.结果 23例患者近期疗效良好.CR 11例,肿瘤消失,管腔通畅;SR 9例,6例仅见有少许肿物残留,3例肿瘤缩小一半以上;MR 2例,因肿瘤堵塞管腔无法插入光纤所致;1例隆突转移癌PDT治疗后第3天因肿瘤坏死脱落致大出血而死亡.结论 PDT治疗支气管肺癌具有疗效确切、副作用小、可重复治疗的特点,尤其对不能手术的气管、支气管腔内生长为主的中央型肺癌、支气管肺癌术后残端复发及有放、化疗禁忌证的患者具有较好的临床应用价值.

Clinical analysis of bronchial lung carcinoma treated with photodynamic therapy

Objective To explore the cli nical therapeutic effectiveness of photodynamic the rapy (PDT) in treating bronchial lung carcinoma.Method The lesion site was irr adiated with German CERALAS PDT 630 Laser 200 J/cm 48 hours after intraveno us dr ip of the German photosensitizer photosan-3. Repetition was done within 72-96 ho urs if necessary.Results Twenty-three patie nts received the PDT treatment and the early e ffect is satisfactory.Eleven cases showed CR with disappearance of the tumor and unobstructed bronchus.Nine cases showed SR and 6 cases had little remainder of the tumor which was equivalent to half of the original size.Two cases showed MR resulting from that the optical fiber could not be inserted because of the obstruction by the tumor.One case with metastasis to tracheal prominence died of heavy bleeding 3 days after PDT tre atment. ConclusionS Treating bronchial lung carcinoma with P DT has the advantage of definite efficacy and few side effects,and the treatment can be repeated. It is of clinical app licable significance and suitable for patients with central-type lung cancer of inoperable trachea, bronchi or postoperative residual relapse or for patients with chemotherapy and radiotherapy contraindications
