氩氦刀并三维适形放疗对较大体积非小细胞肺癌效果

摘    要:目的探讨氩氦靶向冷冻治疗系统(氩氦刀)结合三维适形放疗治疗较大体积肺癌的效果。方法选择CT影像肿瘤直径≥5cm的非小细胞肺癌26例为研究对象,其中鳞癌18例,腺癌8例;Ⅲ期17例,Ⅳ期9例;中位年龄54岁。先行氩氦刀经皮穿刺冷冻治疗,1~2周后行三维适形放疗,DT为50~65Gy,每次1.8~2.0Gy,常规分割。结果所有入组病人均完成治疗计划。CT复查,完全缓解4例,部分缓解22例,总有效率为100%。结论较大体积的非小细胞肺癌以氩氦刀联合三维适形放疗治疗能提高局部控制率。

Abstract:Objective To investigate the efficacy of cryocare therapy combined with three-dimensional radiotherapy(3-DRT) for large non-small-cell lung cancer(NSCLC). Methods Twenty-six NSCLC patients,a median age of 54,with the size of cancer of ≥5 cm in diameter were enrolled,of whom,18 with squamous cell cancer,and eight with adenocarcinoma.There were 17 patients at stage Ⅲ,nine at stage Ⅳ.After one-to two-week cryotherapy,3-DRT was then followed by DT 50—65 Gy,1.8-2.0 Gy per radiation.Conventional fractionated radiation therapy was carried out. Results All the patients had completed the treatment plan.CT follow-up review showed that complete remission(CR) in four patients,partial remission(PR) in 22,with a total effective rate(CR+PR) of 100%. Conclusion Cryocare combined with thee-dimensional radiotherapy can better raise the local control rate of larger NSCLC.
