摘    要:呼吸机相关性肺炎(VAP)发生率高,可增加患者病死率。呼吸机管路的管理是预防VAP的策略之一。更换呼吸机管路的间隔时间与VAP的关系尚不十分明了,上世纪60年代、80年代和90年代初期推荐更换呼吸机管路的间隔时间分别是8h、24h和48h。后来的研究发现,延长呼吸机管路更换间隔时间并不增加VAP发生率。自20世纪90年代中期,呼吸机管路更换间隔时间延长到7d甚至更长,但呼吸机管路细菌定植与VAP的关系尚待进一步研究。

Ventilator circuits change and ventilator - associated pneumonia

Abstract:Ventilator - associated pneumonia (VAP) keeps high frequency and results in increase in mortality. Management of ventilator circuit changes has become the strategy to prevent VAP. The relation between interval of changing ventilator circuits and VAP is not very clear. Changing ventilator circuits every 8 hours, 24 hours and 48 hours were recommended in the 60' s, 80' s and early 90' s in the last century, respectively. Later studies found no increase in VAP with more extended circuits change interval. This made the interval of changing ventilator circuits come to 7 days, or even more since the mid - 90' s of the 20^th century. The correlations of circuits change interval and bacterial colonization of circuits, bacterial colonization and VAP, need more researches.
