卵裂球的存活状况对冻融胚胎种植率的影响

摘    要:目的:观察冻融胚胎卵裂球存活状况与冻融胚胎种植率的关系. 方法:对134个冻融胚胎移植周期进行回顾性分析. 根据冻融胚胎移植周期中卵裂球存活状况分为3组:A组(n=46)为所有移植胚胎卵裂球全部存活;B组(n=65)为混合移植;C组(n=23)为所有移植胚胎均有卵裂球受损. 结果:在134个FET治疗周期中,共解冻胚胎642个,复苏胚胎455个. 有33个周期获临床妊娠,每个冻融胚胎移植周期的临床妊娠率为24.6%. A组的临床妊娠率和种植率为34.7%和18.5%,B组的临床妊娠率和种植率为23.1%和8.2%,C组的临床妊娠率和种植率为8.7%和3.8%. A组种植率明显高于C组(P《0.05). 随着移植胚胎中含100%卵裂球存活胚胎数的增加(0,1,2,3),其临床妊娠率(8.7%,15.0%,31.4%,38.9%)和种植率(3.8%,6.1%,16.2%,18.5%)均明显增加(P《0.05). 结论:卵裂球的存活状况是影响冻融胚胎种植率的重要因素.

Abstract:AIM: To study the impact of blastomere survival on the implantation rate of frozen-thawed embryo transfer FET.METHODS:We conducted a retrospective analysis of 134 FET cycles from September 2003 to September 2005.According to blastomere survival rates,all FET cycles were categorized into 3 groups: group A(n=46) included cycles in which all their original blastomeres remained intact;group B(n=65) refered to mixed transfer of fully intact embryos together with partially damaged ones;group Cn=23 was composed of all transferred embryos with partially damaged blastomere.RESULTS: Out of 642 thawed embryos,455 survived,survival rate being 70.8%.The pregnancy rate was 24.6% per FET cycle.The pregnancy rate and the implantation rate of group A,B,and C were respectively 34.7% and 18.5%,23.1% and 8.2%,8.7% and (3.8%).There was a significantly higher implantation rate in group A than in group C(P<0.05).Along with the increase of intact embryos in FET cycles(0,1,2,3),there was a significant increase in the pregnancy rate(8.7%,15.0%,31.4%,38.9%) and the implantation rate(3.8%,6.1%,16.2%,(18.5%,) P<0.05).CONCLUSION:Blastomere survival status is a most important factor influencing the implantation rate of frozen-thawed embryo transfer.
