瞬目反射与肛门括约肌肌电图对帕金森病患者病情分期判定的意义

目的探讨瞬目反射与肛门括约肌肌电图(EAS-EMG)检查对帕金森病(PD)诊断及病情分期判定的意义。方法选择PD患者108例(PD组),其中早期患者41例,中、晚期患者67例;健康对照组为同期50例健康体检者。分别对2组行瞬目反射与EAS-EMG检查。应用SPSS 12.0软件对其结果进行统计学分析。结果 PD组瞬目反射R2、R2'波潜伏期均较健康对照组明显延长(P<0.01,P<0.05);中、晚期PD组瞬目反射R2、R2'波潜伏期较早期PD组延长(P<0.05)。放松状态下,EAS-EMG检查显示PD组出现自发电位3例,且均为中、晚期患者;PD组与健康对照组比较运动单位动作电位(MUP)的平均时限、多相波的百分比差异均有统计学意义(P<0.01);中、晚期PD组与早期PD组比较MUP的平均时限、多相波的百分比差异均有统计学意义(P<0.05)。结论瞬目反射与EAS-EMG可以作为PD诊断、病情分期判定的客观观察指标。

Significance of blink reflex and anal sphincter electromyography in judging the stage of patients with Parkinson′s disease

Objective To investigate the significance of blink reflex and external anal sphincter electromyography(EAS-EMG) in the diagnosis and judging the stage of patients with Parkinson′s disease(PD).Methods A total of 108 patients with PD were selected as PD group,including 41 patients in the early stage and 67 patients in the late or advanced stage.Fifty healthy physical examination people were selected as healthy control group.Blink reflex and EAS-EMG were examined in the two groups,and the result was analyzed through SPSS 12.0 software.Results Blink reflex R2,R2′ wave latency in PD group extend significantly compared with those in healthy control group(P<0.01,P<0.05).Blink reflex R2,R2′ wave latency in early stage group extend significantly compared with those in late or advanced stage group(P<0.05).Spontaneous potential were observed in 3 cases of late or advanced stage group in relax state.The differences of average time percentage and multiphase wave percentage of motor unit action potentials(MUP) between PD group and healthy control group were statistically significant(P<0.01).The differences of average time percentage and multiphase wave percentage of MUP late or advanced stage group and early stage group were statistically significant(P<0.05).Conclusion Blink reflex and EAS-EMG could be the objective observation indicators of diagnosis and disease stage determination in PD.
