颈肩部压痛的测定及其在神经根型颈椎病诊疗过程中的意义

目的评价应用人体压痛力学定量测试仪测定颈肩部压痛方法及其对旋提手法疗效的意义。方法本组神经根型颈椎病患者210例（脱落12例）,其中试验组106例（脱落5例）,对照组104例（脱落7例）。治疗方法试验组采用旋提手法,对照组采用牵引疗法,应用人体压痛力学定量测试仪于治疗前和治疗后第1、3、5、7、9、11、13天及治疗后1个月随访8个时间点测量颈肩部压痛。结果210例患者常见压痛点有棘突旁、肩胛骨上角、斜方肌颈肩移行部、肩胛骨内侧缘等。第3、4、5、6、7次复诊和1月随访试验组颈肩部压痛缓解优于对照组（P〈0.05或P〈0.01）。结论人体压痛力学定量测试仪有利于压痛的量化评定,旋提手法可明显减轻神经根型颈椎病患者的颈肩部压痛。

The determination for tenderness of neck and shoulder,benefits to diagnosis and treatment of cervicalspondylotic radiculopathy

Objective To evaluate the measurement for tenderness of neck and shoulder with test meter of tenderness,and curative effect of rotation-traction manipulation. Methods 210 patients （12 ablated）with cervical spondylotic radiculopathy were divided randomly into two groups （testing group n=101 ,control group n=97 ）. Testing group was treated with rotation-traction manipulation,control group was treated with neck traction,and were measured tenderness of neck and shoulder with test meter of tenderness at pretherapy,l.3.5.7.9.11.13 day post-treatment and 1 month follow-up visit. Results The common tenderness points located at the side of processus spinosus.medial angle of scapula.neck and shoulder transmigration of cowl-muscle （upper part of trapezias muscle）.margo medialis scapulae. At 3rd.Sth.7th return visit and 1 month follow-up,the tenderness of neck and shoulder of testing group relieved more than control group（P〈0.05,P〈0.01）. Conclusion The test meter of tenderness can benefit to evaluate quantitily tenderness,rotationtraction manipulation in treating cervical spondylotic radiculopathy can improved tenderness of neck and shoulder.
