人工髋关节置换术对老年股骨颈骨折术后功能及临床疗效的影响

目的比较人工股骨头置换术(FHA)、全髋关节置换术(THR)治疗老年人股骨颈骨折的临床疗效及患者满意度。方法采用人工股骨头置换术、全髋关节置换术各治疗股骨颈骨折患者96例,观察并记录临床疗效、患者满意度及手术时间、术中失血量、卧床时间、住院时间。结果治疗后优秀率THR组为54.17%,FHA组为38.54%;2组相比差异有统计学意义(P0.05);优良率THR组为87.50%,FHA组为72.92%,2组相比差异有统计学意义(P0.05)。THR组卧床时间、住院时间短于对照组,2组相比差异有统计学意义(P0.05);THR组术中出血量、手术时间2组相比差异无统计学意义(P0.05)。治疗后满意率THR组为87.50%,FHA组为78.65%2组相比差异有统计学意义(P0.05)。结论 THR与FHA相比,THR治疗股骨颈骨折较临床疗效好,患肢术后功能恢复较好,创伤较小,出血量较少,手术时间较短,且患者满意度高。

【Objective】To compare the artificial femoral head replacement,total hip arthroplasty for old femoral neck fracture treatment efficacy and patient satisfaction.【Methods】hemiarthroplasty,total hip arthroplasty for femoral neck fracture of all 96 patients were observed and recorded the clinical efficacy,patient satisfaction and operative time,intraoperative blood loss,bed rest,hospitalization time.【Results】After treatment,excellent rates of THR group was 54.17%,FHA group,38.54% ;2 groups was statistically significant difference compared（P〈 0.05） ;good rate THR group was 87.50%,FHA group was 72.92%,group 2 with significantly higher than（P〉 0.05）.THR group bed time,shorter hospital stay than the control group,two groups was statistically significant difference compared（P〈 0.05） ;THR group blood loss,operative time compared with 2 groups showed no significant difference（P〈 0.05）.THR satisfaction rate after treatment group 87.50%,FHA group compared to 78.65% 2 difference was significant（P〈 0.05）.【Conclusion】THR compared with the FHA,THR femoral neck fractures clinical curative effect,limb functional recovery better,less injury,less blood loss,shorter operative time,and high patient satisfaction.
